Prif amcan y Gwasanaeth Cymorth Mewn Gwaith (IWS) yw cefnogi pobl gyflogedig a\nhunangyflogedig sydd \u00e2 chyflwr iechyd meddwl neu gorfforol i'w helpu i aros mewn gwaith.<\/p>

Er mwyn ein galluogi i'ch cefnogi drwy'r IWS, mae angen i ni ofyn i chi ddarparu ystod o wybodaeth\ncategori personol ac arbennig i'n proseswyr data: -<\/p>

    Byddant yn darparu cefnogaeth therapiwtig i helpu i wella cyflwr eich iechyd i'ch helpu i aros mewn\ngwaith neu i ddychwelyd i'r gwaith yn gyflymach os ydych yn absennol o'r gwaith oherwydd cyflwr\neich iechyd. Mae hyfforddiant Lles yn y Gweithle hefyd ar gael i fusnesau (busnesau micro, bach a\nchanolig).<\/p>

    Ar \u00f4l derbyn y wybodaeth gan y darparwyr cyflenwi, Llywodraeth Cymru fydd rheolwr data ar ei gyfer. Yn ein cylch gwaith fel y rheolwr data, mae Llywodraeth Cymru yn defnyddio'r wybodaeth a dderbynnir at y dibenion isod. Mae'r dibenion hyn yn angenrheidiol er mwyn ein galluogi i gyflawni\nein tasg gyhoeddus (erthygl 6(e) ac wrth arfer ein hawdurdod swyddogol ac erthygl 9(g).<\/p>

    The information that we request from you will be used to demonstrate eligibility and to evaluate the demographics of individuals supported and whether the service has been effective in achieving its purpose, and efficient in how it produced results. The information will also be used by the Welsh Government for the purpose of administrating and managing the project and allow us to support you through the IWS, to measure and evaluate service performance, and to strengthen the evidence basis for therapeutic support provided to people with mental and physical health conditions and Workplace Wellbeing support to businesses (micro, small and medium sized enterprises). Information about how the support helped the recovery of your health condition may also be shared with your GP Practice, where consented for the delivery provider to discuss your case with your GP Practice, and if you chose to be supported at a GP Practice.<\/p>
